We are currently seeking dedicated Registered Nurses (RNs) to join our professional team and provide outstanding in-home care to clients in need.
Position Overview
We are looking for compassionate, reliable, and dedicated Registered Nurses (RNs) to provide skilled nursing services in the homes of our clients. As an RN, you will assess patient health, develop care plans, and supervise home health aides to ensure exceptional care. You will work collaboratively with a nurse-led team, empowering patients to maintain their independence and improve their quality of life in the comfort of their own homes.
Responsibilities

Conduct patient assessments, including health evaluations, medical histories, and physical exams.

Develop and implement personalized care plans based on the patient’s health status, preferences, and needs.

Administer medications and treatments as prescribed, and monitor for side effects or adverse reactions.

Monitor and document patient progress, including vital signs, symptoms, and any changes in their condition.

Provide wound care, catheter care, and other skilled nursing interventions.

Supervise and mentor Home Health Aides (HHAs) and other caregivers to ensure proper care is being provided.

Educate patients and families on disease management, health maintenance, and home care procedures.

Communicate and coordinate care with physicians, other healthcare professionals, and family members.

Document all medical records accurately and in compliance with regulatory standards.

Respond to emergency situations and provide care as necessary.

Maintain the highest standards of safety, cleanliness, and infection control.
Qualifications

Active Registered Nurse (RN) license in the state of Virginia.

1+ years of clinical experience in a home health, hospital, or long-term care setting.

Strong critical thinking and clinical decision-making skills.

Ability to develop and manage care plans based on patient needs.

Excellent communication and interpersonal skills.

Ability to lead and supervise a team of caregivers (HHAs, CNAs, etc.).

Valid driver’s license and reliable transportation.

CPR/BLS certification required.

Must be able to pass background checks and provide a negative TB test.
